Raider Athletes and Parents:
Greeting from the Athletic Training Staff of Southern Oregon University. Please take a moment to review this letter and the materials that make up your Athletic Participation packet.
All enclosed information must be completed and returned to the Athletic Training Staff before you will be allowed to participate in any athletic practices or competitions.
Southern Oregon University policy requires that all student athletes must have primary health insurance while participating in-season. The Insurance Questionnaire form includes a request for a copy of your medical insurance card. This can be attached to the form or copied separately. This information will be kept confidential in each student’s medical record file. Please read all forms carefully before signing.
Every new incoming Raider athlete must have a physical examination to be allowed to participate in any athletic practices or competitions. All physical examinations must be completed on SOU’s official physical form and signed by a licensed medical doctor (MD or DO). If not completed before the first day of reporting, they will be a $15 charge to schedule a physical during the first day of reporting.
Please take time to fill out all of these forms as thoroughly as possible. When completed, mail all components of your Athletic Participation packet to the attention of the Athletic Training Staff or bring during the first day of reporting. Please call with any additional questions.
